In 3 simple steps:

On your next appointment or at any time, please visit our reception and ask for your registration leaflet.
Attend the surgery with some photo ID the receptionist will give you a letter with your personal PIN information and instructions for your registration.

For security reasons this registration Letter can only be obtained personally.

  • Patients under 16 will need to have a parent or guardian to register for them.

patient Access letter

Important. To start the registration process online you need the letter with your personal PIN information and instructions for your registration.

You don’t have one yet?. Try Register Today Without the Registration Letter.

1. Once you have your registration letter visit www.yhs.care/register or click the “Sign In” button on the top right of the web site.
The register screen will  open in a new tab in your internet browser.
2. Click on the Register button.
3. If you got the letter from the York House Surgery please click Yes, if you don’t have one please go to the top of this page and read the instructions to get one.
4. Enter the registration details in the letter and click Next. Remember, the information in the letter is personal and nontransferable, if your letter is lost or stolen before completing the registration process, please contact your practice immediately.
Once the registration process is finished, please destroy the letter.
5. Enter the information required, the password must be at least 7 characters long and must contain Capitals, lowercase and numbers to be a strong password. Then click Next.
6. Check your Contact Details, and select the security questions.
Security questions are used in case you lose you password or ID. In that case during the process to recover your ID or retrieve a new password the system will display the security questions that you chose and you must to enter the right answer to complete the process, the security questions and the answers are private.
Read and tick the terms and conditions box, and click Next.
7. Congratulations you have now successfully created an account.

An email will be send yo your email account with your new ID number, the email subject is: Patient Access Registration Submitted (in case need search for your ID at any moment).

It is very important that you keep a copy of your ID number and don’t forget your password.

Now click on the “Sign in” button to access the Patient Access System.
Every time that you want to access the Patient Access System you will need your ID number and the password that you choose in the step 5.
